Free Youth Tennis Lessons

By submitting this form, I give permission for my child to participate in the activities listed. I hereby release and hold harmless My Hometown and Liberty Hills Tennis from all liability, claims, damages, and demands of whatever nature, either in law or in equity, which arise or may hereafter arise from participation in this activity. I understand and acknowledge that this Release discharges My Hometown and Liberty Hills Tennis from any liability or claim that I may have with respect to any bodily injury, personal injury, illness, death, or property damage that may result from my participation in this activity.

Parents and participants should understand that participation in this activity is not a right but a privilege that can be revoked if participants behave inappropriately or if they pose a risk to themselves or others.

I also give My Hometown permission to take and use photographs/video of my child for any legal use, including, but not limited to: publicity, illustration, advertising, and web content. Furthermore, I understand that no royalty, fee, or other compensation shall become payable to me by reason of such use.
